We first met NCIS Special Agent Paula Cassidy (played by Jessica Steen) in 1.08 "Minimum Security." According to IMDB, she was also in 1.16 "Bete Noire," 1.23 "Reveille," 2.08 "Heart Break," and 3.03 "Mind Games."
Paula Cassidy's final episode was 4.19 "Grace Period."
Synopsis: "When a weekend call to the NCIS tip line from an unknown source about possible terrorist activity results in the death of two agents sent into a trap, the NCIS team will have to find out who was responsible, with the help of Agent Paula Cassidy, who holds herself responsible for the tragedy."
I hope that helps jog your memory about how her character came to an abrupt demise at the end of "Grace Period." It was so sad!
